LJP Lab will bill insurance carriers if all required information is provided. It’s important to note each patient is responsible for all unpaid balances for all completed laboratory services.
LJP Lab will bill Medicare directly, and accept the designated assigned cost for all covered services. It is important to note, all toxicology tests are not covered by Medicare insurance. In this instance, the patient will be informed prior to any testing, and must sign an ABN. An ABN states the patient knows the service may not be covered, and they are responsible for any subsequent charges.
Worker’s compensation claims will be filed if all required information is provided at the time the specimen is submitted for analysis.
LJP Lab will provide patients or their attorneys with all records and copies of the bill. However, in the case of personal injury, all tests must be ordered by a physician and the patient must prepay at the time the specimens are submitted for testing.
